  • Coverage Dates and Start Date (Specific, long term only): ASAP – July 24, then potentially ongoing through fall
    
  • Shift Schedule Details & Shift Duration: 8a-8:30p or 9a-9:30p with 30-minute lunch
    
  • Minimum length of commitment requirement? 1 shift per week through July
    
  • Patients per Day/Volume: 35
    
  • Type of Patients/Cases: Peds, adults - UC
    
  • Support Staff: other doctors and APP on site
    
  • EMR: EPIC – prefer already proficient
    
  • Certifications/Qualifications Required: BLS
    
  • Board Certified Required: Yes – board certified in field of training
    
  • IMLC State: No
    
  • Active State License Required? OR
    
  • CSR/DEA Requirements (State or Client): OR
