Job Description Setting/n& Population Served /n/n Life Care
Centers of America facilities operate as Skilled/nNursing
Facilities (SNFs) that provide: /n/n /n - Long term care for
residents who require/n ongoing skilled support and compassionate
daily assistance /n - Short term, sub-acute rehabilitation for/n
patients recovering from surgery, illness, or injury /n -
Collaborative care through an/n interdisciplinary team approach
with nursing, therapy, and medical staff/n working together /n - A
diverse patient population including/n individuals with orthopedic,
neurological, cardiopulmonary, and post-acute/n needs /n /n/n/n/n
Patient mix varies slightly by facility. As a
therapy/nprofessional, you will help patients achieve functional
progress and improve/nquality of life within a supportive,
patient-centered environment. Position Summary/n The COTA Certified
Occupational Therapist Assistant provides direct rehab care in
occupational therapy to patients upon physician referral under the
supervision of a licensed Occupational Therapist (OT) in accordance
with all applicable laws, regulations, and Life Care standards.
/nEducation, Experience, and Licensure Requirements/n /n - NBCOT
certified upon hire, but renewal is optional going forward /n -
Graduate of an accredited program in occupational therapy (ASOT) /n
- Currently registered/licensed in applicable State. Must maintain
an active license in good standing throughout employment. /n - One
(1) year experience in post acute care or related setting preferred
/n - CPR certification upon hire or obtain during orientation. CPR
certification must remain current during employment. /n /nSpecific
Job Requirements/n /n - Participates in community health
matters/projects as appropriate /n - Proficient in Microsoft Word,
Excel, and e mail /n - Liaisons with patients, families, support
departments, etc., to adequately plan for patient needs /n -
Demonstrate good body mechanics at all times /n - Make independent
decisions when circumstances warrant such action /n - Knowledgeable
of practices and procedures as well as the laws, regulations, and
guidelines governing functions in the post acute care facility /n -
Implement and interpret the programs, goals, objectives, policies,
and procedures of the department /n - Perform proficiently in all
competency areas including but not limited to: patient rights, and
safety and sanitation /n - Maintains professional working
relationships with all associates, vendors, etc. /n - Maintains
confidentiality of all proprietary and/or confidential information
/n - Understand and follow company policies including harassment
and compliance procedures /n - Displays integrity and
professionalism by adhering to Life Care’s Code of Conduct and
completes mandatory Code of Conduct and other appropriate
compliance training /n /nEssential Functions/n /n - Follow
occupational treatment plans for patients under the direction of
the supervising OT /n - Chart appropriately and timely /n - Utilize
therapy software appropriately and accurately /n - Exhibit
excellent customer service and a positive attitude towards patients
/n - Assist in the evacuation of patients /n - Demonstrate
dependable, regular attendance /n - Concentrate and use reasoning
skills and good judgment /n - Communicate and function productively
on an interdisciplinary team /n - Sit, stand, bend, lift, push,
pull, stoop, walk, reach, and move intermittently during working
hours /n - Read, write, speak, and understand the English language
