Job Description:
Kaiser Permanente Georgia is seeking a dedicated ED RN Care
Coordinator to enhance our Emergency Department team by delivering
optimal care coordination, fostering seamless coverage, and
improving the health outcomes of our members. In this pivotal role,
you will collaborate with multidisciplinary teams to provide
high-quality, patient-centered care, ensuring that care delivery
aligns with our commitment to excellence in health services.
Coordinate patient care across different healthcare services and
levels within the emergency department. Assess patient needs and
develop care plans in collaboration with other healthcare
professionals. Act as a liaison between patients, family members,
and various healthcare providers. Ensure that documentation and
care plans are completed accurately and in a timely manner.
Contribute to the continuous improvement of nursing practices and
patient health outcomes. Ability to coordinate complex patient care
in a high-stress environment. Strong communication and
interpersonal skills. Proficient in electronic health record (EHR)
systems. Aptitude for detail and precision in case documentation.

Certification in Basic Life Support (BLS) and Advanced Cardiac Life
