Competitive pay, scenic Pacific Northwest location, and opportunities for both travel RNs and local Oregon nursing job candidates. /n/nHome Health Travel Job Details/n /n - Weekly Estimated Pay: $2,390 – $2,520/n - Location: Roseburg, OR (Travel radius up to 75 miles from clinic) — Oregon nursing jobs / Roseburg healthcare jobs/n - Assignment Duration: 13 weeks (Travel Contract)/n - Shift: Day shifts, Monday–Friday, 8:00 AM – 5:00 PM (8-hour days)/n - Hours per Week: 40/n - Employment Type: Travel Contract (Home Health RN / Travel RN)/n - Patient Load: Average 5–8 patients/day/n - Productivity Expectation: 30 points/week/n - Dress Code: Scrubs/n - EMR: HomeCare HomeBase (HCHB)/n - OASIS proficiency required/n - Guaranteed Hours: No (opportunity to float to nearby clinics if needed)/n - Weekend/On-Call: May vary; discuss during interview/n - Local candidates accepted/n /n/nRequired Qualifications/n /n - Minimum 1 year of recent Adult Home Health or Hospice RN experience (Home Health Nurse or Hospice RN)/n - Active RN license in Oregon (must have license in hand)/n - BLS/CPR certification required/n - Proficient in HomeCare HomeBase (HCHB) EMR and OASIS (used within last 6 months or willing to complete retraining)/n - Reliable vehicle, valid driver’s license, and agency-required liability insurance/n - Comfortable with travel up to 75 miles from home clinic (travel RN / home health travel)/n - Ability to complete documentation within 24 hours of patient visit/n - Open to on-call rotations and accepting all scheduled patients/n /n/nPrimary Responsibilities/n /n - Coordinate and direct home nursing care according to physician orders and agency policies/n - Conduct home visits for patient care, including Start of Care (SOC), Wound Care, Follow-up Visits, and Recertifications/n - Supervise and evaluate Home Health Aides and LPNs/LVNs/n - Perform admission assessments, case management, recertification, and discharge planning/n - Provide wound care, medication management, pain assessment, and symptom management/n - Manage IVs, catheters, feeding tubes, and other clinical procedures as needed/n - Ensure timely and accurate documentation using HomeCare HomeBase and OASIS (clinical documentation, point tracking)/n - Participate in periodic office meetings and collaborate with the interdisciplinary care team/n - Maintain infection control precautions and adhere to agency protocols/n /n/n Join our dedicated home health team in Roseburg, Oregon and make a meaningful impact as a Homecare RN.
