Job Description SummaryThe medical social worker provides direct casework and counseling services to the patient/family unit. The social worker provides diagnostic evaluation of psychosocial problems and defines social service goals for alleviating problems.Essential Job Functions/Responsibilities Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient's plan of care. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required. Demonstrates exceptional service to patients, families, referral sources, co-workers and other customers. Assists team members as needed to ensure that this level of service is consistently provided Counsels patient and family/caregivers and staff as needed in relationship to stress, and other identified coping difficulties Provides accurate and timely documentation of patient services to reflect the plan of care Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs. Serves as liaison between patients and families/caregivers and community agencies. Maintains collaborative relationships with organization personnel to support patient care. Maintains and develops contracts with public and private agencies as resources for patient and personnel. Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings. Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues. Actively participates in quality assessment performance improvement teams and activities. Requests spiritual support of patients/families, as needed. Provides appropriate intervention for high-risk patients/families/survivors that hare experiencing suicidal ideation. Communicates with hospitals, doctors, and other health care providers regarding existing and potential billing problems and coordinates efforts to resolve them. Develops and maintains close working relationship with long-term care facility staff. Is available for on-call. The incumbents may be requested to perform job related tasks other than those stated in this description.Position QualificationsA graduate of a bachelor's program in social work accredited by the Council on Social work Education, in state's that allow a licensed holder of a Baccalaureate Degree in Social Work, to practice in a hospice setting under the supervision of a Master's prepared and licensed social worker. A graduate of a master's program in social work accredited by the Council on Social Work Education in state's that require a Master's Degree in Social Work to practice in a hospice setting. Minimum of one year's experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying. Experience in a hospice care preferred. Demonstrates good verbal and written communication, and organization skills. Maintains a current CPR certification. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Demonstrates excellent observation, problem solving, verbal and written communications; skills per competency checklist. Shows ability to organize and prioritize workload independently. Works indoors in patients' homes. May be required to work in undesirable areas. Possibility of strains due to moving patients.Continuing Education Requirements: Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. This position requires a minimum of 8 education hours. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs.ENVIRONMENTAL AND WORKING CONDITIONS Environmental Conditions: May be exposed to extremes of heat and cold in all weather conditions. May be required to drive in various weather conditions on roads in varying degrees of repair. Working Conditions:May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed or occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA exposure category: Category I - Position includes tasks that involve exposure to blood, body fluids, tissues. Required Personnel Protective Equipment: As required by working conditions.
