ShiftDayDescriptionA health care professional who works in
partnership with primary care providers, behavioral health
professionals, community partners, patients and patient family
members to ensure the best possible outcome for the patient. The
PCC primary role is to facilitate and coordinate patient care and
back office work flow to be as efficient and effective as possible.
This is a 5 day a week, 8 hour/day position. Job duties of this
position will include but are not limited to the following: Create
an efficient work flow for all team activities and manage daily
team huddles to discuss scheduling/pt. flow needs.Thoughtfully and
accurately schedule patients using a scheduling algorithm to ensure
proper appointment placement and time needs. Facilitate appropriate
case management, tracking and follow up for all patient care.
Follow up and track care on high risk patients.Utilize recall list
protocol to facilitate open access scheduling.Coordinate Behavioral
Medicine visits for chronic conditions and new patients in
conjunction with provider schedule. Attend appropriate meetings to
facilitate and report on ongoing position updates. Review schedule
and initiate chart reviews to ensure preventative health is up to
date. Triage as needed. Fill in as needed for the Medical
Assistants. Be available to assist the Medical Providers as needed
in the clinic.Monitor and follow up of provider referrals,
diagnostic testing and ER/hospital follow ups Call or send patients
reminders of post-dated diagnostic testing as needed. Dataload
health information from outside resources.QualificationsEducational
requirements: LPN licensure preferred for this position but not
required. Must be minimally CCMA with high level work experience.
Qualifications: Self-motivated.Experience scheduling
patients.Experience electronic medical record usage and
documentation.Ability to use Word and Excel.Ability to use a wide
range of office machines, ie PC, fax, copier, and multiline
phone.Problem solving abilities.Ability to demonstrate basic
nursing clinical skills, ie providing injections, vital signs and
dressing changes.Ability to work at a rapid pace, and multitask as
needed.Enthusiasm for the philosophy and mission of the clinic.Team
player.Tact and graciousness dealing with patients and their
families.Current Oregon Driver's License.Current CPR
certification.
